Pain Points & Proposed Use Case

Use Case Definition

Steps in the Visit Preparation

Process

Nurse signs into VPN from

home or office. Once on
VPN, nurse can access
CDS to view previous visit
info, care plan and contact
details.

After review of CDS, nurse

schedules/confirms
appointments and collects
key contacts for the
patient. Currently a
manual process (write
down address, enter in
Google Maps)

Nurse prepares consent

Nurse obtains patient
forms to be signed by
information from CDS and
patient and prints leaveEpic, and prepares visit
info and documentation in behinds and other patient
education materials
MS Word by manually
needed for the visit.
writing, or by typing and
printing it out.

Pain Point Prioritization

High Priority Pain Points
Limited visibility
into traffic and
route conditions

Slow computers
and software take
a long time to sync

No secure way to
send patient
photos

Changing
documentation
requirements

Communication
with care team is
difficult across
follow ups

Lower Priority Pain Points

Process for
Duality of current
contacting patients
systems (KPHC and
and scheduling
CDS)
visits is inefficient

Poor remote
system
connectivity

Documentation
takes a long time
and is frustrating

Unable to access
KP systems on a
mobile phone
