Support to the

Transforming Care Programme


What you need to do!

Remember this picture? This shows how


this work needs to happen.

The National Forum and the


National Valuing Families Forum
have agreed to do some extra
work to support the Transforming
Care Programme.

You will work in your region to collect information from


people with learning disabilities and family carers
about how the local areas are doing in making
Transforming Care happen.

Inclusion North will collect the information from the


regions and send it to the Transforming Care
Assurance Board.

This will mean the Transforming Care


Assurance Board will get really good information
directly from people with learning disabilities and
family carers about how things are working, as
well as from local authorities and Clinical
Commissioning Groups.

There is money to support this work. Each region


will get 5000 to share between the National Forum
and the National Valuing Families Forum.

Some regions are planning to work


together to make the money go
further.

There will also be support from Inclusion North.


We can help you with making your plan about how
to collect information. We can help with materials
you might need for events of meetings.

Inclusion North have worked with the National Forum and


National Valuing Families Forum to agree what
information you need to be collecting in your regions.
You can get a checklist from Inclusion North of this
information.

You need to have your plan ready


by the end of January 2016

Inclusion North needs to send reports to the


Transforming Care Assurance Board. This means that
you need to let them have the information you are
collecting on the checklist that Inclusion North give
you.

Inclusion North needs to send the first report at the beginning of April
2016, then at the beginning of July and the beginning of October.
