AFFIDAVIT OF LOSS
I, ANNA MARIA CERDA CONFIADO, of legal age, Filipino citizen,
with postal address at No. 13 Friendship Rd., Friendly Village I, Concepcion,
Marikina City, after having been duly sworn to in accordance with law, do
hereby depose and state, that:
1.

I am a Certified Public Accountant with License No. 98330;

2.

Sometime 2005, after having lunch at a restaurant at Paseo

Center, I noticed that my hand bag was missing;

3.

The said bag contains all my personal identification cards

including my professional identification card issued by the
Professional Regulation Commission;

4.

I religiously and patiently look for my wallet containing the

above-mentioned items but much to my effort I failed. The same
are now considered lost and beyond recovery;

5.

I am executing this affidavit to attest to the truth of the

foregoing statements, and in whatever legal purpose this affidavit
may serve;

6.

Further Affiant Sayeth Naught.
