A serious near miss incident occurred following fall of Hydra bearing

number HR743934. The incident took place when the hydra operator
tried to shift truss weigh 3MT. As soon hydra operator had lifted the
truss 1/2m from the ground the hydra got unbalanced and toppled its left
side.
Fortunately no injury took place only truss got distorted from purlin.
Area was barricaded.
1. Risk assessment of critical activity was not done.
2. Area was not even.
3. lack of supervision for critical job.( Heavy materials handling)
Property damaged:
Purlin got distorted from some places.

Assess the risk before starting critical activities
through risk assessment. Communicate the same
risk to involve working people. Operator must be
competent.
Area where lifting and shifting activities to be
carried out must be even and compact. So that
undesirable incident could be averted.
The job Supervisior must access the risk and be
present through out the job execution.
Two hydras are ban for any lifting and shifting. It
also bans to shift heavy materials by hydra. Use
trailer instead of hydra for shifting of materials.
