Before connection to the unloading window truck driver

stopped truck near window and went to open truck
gate. In the same time, plant FLT drivers who worked
near the gats heard sounds of crashing glass and saw
truck driver seating on the floor. Driver has got injury
displaced fracture of heel bones at both legs.
According to truck driver he received injury from
falling crates. But this type of injuries almost in 100 %
of cases happened after jumping or falling from height
on legs. No witnesses

Corrective and Preventive Measures

To review Risk Assessment for truck opening and removing block-out
pieces (devices to fix cargo)
To add new critical behavior in Golden Rules for truck drivers safe
access into trailer, cascade to truck drivers and companies
To add in safety leaflet requirements for safe access into trailer
To set requirements where driver must stay during opening trailer gate
To install CCTV system to monitor trailers gates opening process
To inform transport companies about all safety violations by truck
drivers
To develop clear procedure how to remove block-out pieces
Communicate to transport companies new safety requirements (review
contracts)
To provide necessary tools (ladder with platform / handrails) for safe
access to trailer (removing block-out pieces / belts )
To review / check ways of fixing cargo with distributors.

Victims unsafe behavior
Lack of Risk assessment for these
activities (entrance into semitrailer,
removing fixing devices from the
semitrailer)
Lack of requirements for the activities in
safety induction, contracts and leaflets
for drivers
Lack of procedure / tools for removing of
fixing devices for cargo.
Lack of safety knowledge from truck
driver.
