Background: Terengganu Stadium roof collapse after one year of being built.

Detail:
Just shortly over a year after it was officially opened by the Yang di-Pertuan
Agong Sultan Mizan Zainal Abidin, the roof of the stadium named after him in
Gong Badak, Kuala Terengganu collapsed this morning, damaging a few cars that
were parked in the vicinity. No one was injured. Police have cordoned off the
roads leading to the stadium. The frame structure of the roof of the 50,000-seat
stadium came crashing down around 7.30 am this morning and none of the 50odd stadium staff were at the premises yet. According to a worker, Mohd Zaidi
Ahmad who was one of two workers who were fixing light sat the stadium about
300m from where the incident happened, said he rushed to the scene to see if
there were any casualties.
However, although there was no immediate news of anyone being injured, a
team from the fire and rescue department and the Civil Defence Unit were at the
scene to check if there were any victims trapped under the rubble.
Causes:
After an initial investigation, I discovered that serious engineering flaws, shoddy
workmanship, inferior materials and lack of expertise in the key project
management team were found to be the main contributing factors behind the
roof collapse. A further investigation revealed that the project management team
also did not have the necessary skills and competence to manage a project of
such magnitude and complexity. Other factors contributing to the collapse were
the undesirable manner in which the structure was erected and that no checks
were done during the erection process
Recommendation:
To prevent similar occurances, I suggest the following:
1. Appointment of a new team of consultants based on a quality-based
selection system.
2. Appointment of a new team of accredited independent check engineers.
3. The repair works should be undertaken by a specialist contractor.
