1. Take the original Passport and CPR copy of the deceased
to Salmaniya Mortuary. 2. Collect Death Notification from form Mortuary. 3. Take the Death Notification to Min of Health Offices neat Batelco Building in Salmaniya with authorization letter from Sponsor 4. Collect Death Certificate from Ministry of Health. 5. Death certificate to be attested from Ministry of foreign affairs in Manama Centre. 6. The Death certificate shall be taken to Indian Embassy with original Passport of the deceased and an undertaking from the Next of Kin (wife/ Son) that all the dues are received from the employer and the body is being repatriated by the sponsor. CPR copy of NOK shall be attached to the undertaking. The CPR copies and PP details of the person authorized to receive the body and accompany the body shall also be given to embassy officials. 7. Undertaking from sponsor (EWA) to embassy regarding the details of the deceased employee and that the dues are cleared . 8. Death certificate shall be attested by Embassy and another death certificate by the embassy shall be taken. 9. Coffin to be booked by paying BD 80/-. The coffin receipt and copies of the attested death certificate to be handed over to Salmaniya Morgue. 10. Pay BD 120/- at the Salmaniya cash counter for embalming. The copy of the receipt to be given to morgue staff for embalming. 11. Book Coffin in the Airlines with NOK accompanying the body (thru M/s Kanoo) (BD 200/ coffin to India) LPO to be given by EWA. 12. Take copies of all the certificates to CID offices in Adliya for stamping. 13. Take police(CID) clearance certificate from Adliya for Airlines cargo loading.
14. Collect the body from Mortuary (after embalming) and
