AFFIDAVIT OF LOSS I, GLENDA PANAGA OABEL, Filipino citizen of legal age, single, and presently residing at NPC Compound Abiong, La Trinidad, Benguet, under oath, states that: 1. I am a PHYSICIAN by profession having earned my medical degree and graduated from Saint Louis University. As such, I was issued my DIPLOMA for my medical degree; 2. Recently, as I was going over my personal files and documents, I noticed that my said DIPLOMA is missing. Thereafter, I exerted diligent efforts to find the same but my efforts turned out to be futile. I can only surmise that the said document was unintentionally misplaced; 3. In view of this circumstance, I now consider my DIPLOMA to be legally lost and beyond recovery; 4. I also declare that my said DIPLOMA was not confiscated for any violation of law, rule or any governmental regulation; 5. That I am executing this affidavit to attest to the truth of the foregoing facts and to request for a replacement of my DIPLOMA, and for all legal intents and purposes it may serve.
