Consent to Participate in Focus Group

What is the Research?

You have been asked to take part in a focus group as part of a research study to determine the best hours of
operation for each Shepeard location (Aiken, SC, and Evans and Augusta, GA). If you choose to participate in
this focus group, you will be asked some questions relating to your experience with Shepeard Community
Blood Center. You will be in a group of approximately 8-20 adults with a facilitator, a recorder and possibly a
few observers.
Voluntary Participation
Participation in this focus group is voluntary.
If any questions make you feel uncomfortable, you do not have to answer them. You may leave the group at any
time for any reason.
Risks and Benefits
Your participation may benefit you and other Shepeard donors by helping improve our availability to donors.
We do not believe any risks are involved in taking part in this study.
Confidentiality
All data gathered in this study will be anonymous and confidential. No individual participant will be identified
or linked to the results in any way. The results of this study will be presented only to Shepeard senior
management. Study records, including this signed consent form will be stored in a secure location at Shepeard
Blood Center and will be available only to the individuals conducting the study. This material will be
destroyed when the study is complete.
Audiotape Permission
This study groups discussion will be recorded for research purposes. Neither your name nor any other
identifying information will be associated with the audio recording or transcript. The recordings will be
available only to the individuals conducting the study.
