Johnny is a grade 8 student, with ODD, IPRCd with Multiple Exceptionalities and
diagnosed ADHD as well as ARND. He is in a regular classroom with indirect support
and is exempt from his Second Language class and works alongside me in a
Communications Class where we focus on life skills, communication and positive
social interactions on a daily basis.

Johnny has been making some effort to transition into the rotary schedule for Grade 8
and with continued monitoring of his progress, he will continue to see some success
in all areas of his learning skills.
Johnny is capable of being responsible and he is able to put forth and show some
initiative when he focused and puts his mind to it. He will continue to make an effort to
collaborate responsibly with his peers and to communicate appropriately with his
teachers in all of his subject areas on a daily basis. With encouragement and support,
Johnny will need motivation to complete daily required tasks and to use his calendar
on a daily basis to help stay organized and on track. Johnnys next steps would be to
ensure that he uses technology appropriately and to use teacher feedback and
support to improve his work and to further his learning.
Johnny has been making some progress in expressing his feelings during the day, to
help communicate how he is feeling before any major conflicts with others arise. He
will continue to make an effort to collaborate responsibly with his peers and to
communicate appropriately with his teachers on a daily basis.
For Term 2, with support, Johnny has set some of his own individual goals and we will
work together towards gradually improving his ability to self-regulate his behaviour by
working 1:1 and provide positive modelling success criteria. Keep up your continued

Behavioural Goals
A) Johnny will demonstrate improved social skills in mainstream classes, such as
being polite and using manners, acknowledging questions asked and asking
open-ended questions, 4 out of 5 days per week, with teacher observation

daily.
B) Johnny will politely ask to use his technology in his classes for educational
purposes and be able to reinstate why it's important to use his technology in a
safe and appropriate manner, with teacher prompts 50% of the time.
C) On a weekly basis, Johnny will perform at least one good deed while at school
(i.e. pick up garbage, hold the door open for someone, make a positive
comment to an adult or peer etc).

Alternative Programming Goals/Success Criteria


By the end of Grade 8, Johnny will have polite interactions with his peers and
teachers, and communicate his feelings 80% of the day. With assistance, Johnny will
choose the best next steps if a conflict arises, and will carry out the suggested steps
and strategies afterwards.
