Behavior 9.1
Select and use appropriate methods for evaluation
of outcomes

- Complete a screening decision guide.


This form is used when a student is referred
to the CFST for intervention. The form is
used to select and determine the
appropriate course of action for the
student.

Behavior 9.2
Apply knowledge of human behavior and the social
environment, person-in-environment, and other
multidisciplinary theoretical frameworks in
evaluation of outcomes

-Complete a Responsiveness to Instruction


Assessment and a Problem Solving Model
Classroom Observation.
-These forms are completed when an observation
of a student is needed to find out where he or
she is having problems in the classroom, that will
help determine appropriate services. These
forms inquire about classroom size, student
behaviors, student-teacher interaction, peer
interaction, response to learning, etc.

Behavior 9.3
Critically analyze, monitor, and evaluate intervention and
program processes and outcomes

-Attend grade level and administration PLC team


meeting
In these meetings I will discuss and provide any data
I have to present to administration or issues that need
to addressed at the administration level. In these
meetings administration will provide myself and
supervisor with anything they wish for us to follow up
on the social worker level, i.e., student abscesses
In grade level PLC meetings teachers collaborate with
the support staff and inform us of meetings or follow
up meetings that are needed with the CFST team

Behavior 9.4
Apply evaluation findings to improve practice effectiveness
at the micro, mezzo, and macro levels

-Follow up on findings and reassessing


interventions
This can be performed in follow up CFST
meetings. The team can point out and
discuss the effectiveness of what has been
implemented. In these meeting goals and
interventions can be readdressed if
necessary.

Behavior 8.5
Facilitate effective transitions and endings that advance mutually agreed-ongoals

-Engage in CFST meetings. These meetings


are implemented for the purpose of
establishing mutually agreed up goals and
interventions that provide an avenue for a
successful student
