Survey Questions: combination of Likert Scale, free-form, and range value from Google Forms.

Scale
and range questions were required, free-form questions were optional. The first 5 questions were based
on the Intel IT Usability Survey.

The questionnaire was done in a spreadsheet for the first 4 Testers, the final 2 received it in email from
Google Forms. The spreadsheet results were entered into the Google forms survey.
