BEHAVIOR

outcomes.
Task and objectives to reach goal: I helped to empower a
resident to seek and improve her living situation at Providence
Place. Application process for long-term living arrangement
closer to her daughter started. Resident lives in Lee County;
her family and support system live in Southern Pines Moore
County.
How I will Measure what I have learned: Resident will have
decreased anxiety about living alone, and far away from family,
decreased out- of pocket expenses for the non-subsidized
apartment she currently resides in.

BEHAVIORS
29. Apply knowledge of human behavior and the social
environment, person-in-environment, and other
multidisciplinary theoretical frameworks in the evaluation of
outcomes.
Task and objectives to reach goal: As individuals change
throughout the spectrum of life many do no realize that
they are no longer able to care for themselves as they once
did. This can cause feelings of disorientation, fear, and
unusual behavior sometimes. Upon arrival at a facility like
SJOP or a health care environment where basic care, food,
nutrition, hygiene and social structure are resumed the
resident one again regain much of their former self.
How I will measure what I have learned: The resident will have
an improvement in self-esteem, self-confidence, feelings of
self-worth and well-being. The resident will exhibit
decrease in anxiety and an increase in physical activity.

BEHAVIORS
30. Critically analyze. monitor, and evaluate interventions and
programs processes and outcomes.
Task and objectives to reach goal: Example 92 y/o resident lives
alone, very independently, recovering from a fractured hip
repair. Resident is alert, awake and oriented x 3, she
maintains her own POA. Her desire is to return to her own
home. Her two adult daughters live 14 hours away and
would like their mother to sell her home and move into longterm care. Intervention suggest alternatives for home care
and physical therapy.
How I will measure what I have learned: Residents two
daughters decided to come to NC to transfer their mother
to her home with private care. They assisted their mother to
determine how long she can afford to maintain this
arrangement. ? Possibly sell the mothers home to finance
her care if she moves in with one of her daughters.

BEHAVIORS
31. Apply evaluation findings to improve practice effectiveness
at the micro, mezzo, and macro levels.
Task and objectives to reach goals: Example an elderly resident
refused to eat meals provided. Family is bringing in
chocolate Ensure from home three x per day. Intervention
dietary called chocolate health shake ordered for the
resident for B, L, D.
How I will measure what I have learned: Increase
communication with the resident, family, and staff in
reference to the decreased potential for weight loss, and
malnutrition by encouraging proper nutrition. Improved
healing, rehabilitation, and the assurance of a continuum of
care with in this facility .
