Our class will be going on a field trip to the Lawrenceburg Fire Department. Please read the information at the top
of this form, then sign and return the permission slip at the bottom of this form by November 13, 2015.

Cost: There is no cost for the field trip.
Means of Transportation: School Bus
Leave school: 9:00am

Arrive back at school: 12:00pm

Special Instructions: If your child would like to purchase a drink from the cafeteria for lunch, please enclose enough
money for them to do so. Otherwise, I will provide them with water. Also, they will need to bring a drink and snack
to have at the firehouse.
