S.A.B. : I complied with the Humber practical nursing mode of dressing, complied with the hand hygiene policy of the hospital setting and
followed all protocols of the practice setting for example monitoring vitals signs before and after blood transfusion taking into
documentation the temperature readings at every 5,10 and 15 mins of infusion time. I always wear P.P.E when going into an isolated room
in order to reduce the spread of nonsocomial infection.

E.W. : Sadiq also complied with the ward policies regarding medication administration, documentation one hour after vital signs, and
following all hip precautions.

S.A.B. : The three factor frame work consist of the client, nurse and environment. Patient assignment is always based on degree of
complexity and stability of a client condition with nursing care shifting from practical nurses to registered nurses as client conditions
becomes unpredictable and unstable. For example, I collaborated with the nursing staff with a client who is on heparin IV and was placed
on an heparin monogram which required frequent aPtt and InR blood readings.

S.A.B. : I do my readings and watch videos about procedure on particular procedures to be done on client. Some of the videos I watched
include dressing change and catheterization. I do research on my medical diagnosis and drugs before administering any medication
especially antihypertensive medications which is required to be held when pulse in less than 60. I actually had to hold a beta blocker
medication because client blood pressure and pulse was low.

S.A.B. : I understand the role of a nurse as an health professional that works to provide ethical and holistic care to client taking in mind
safety of client without causing no harm. A nurse work collaboratively and consultative with different health professionals with the goal of
providing the best therapeutic care for client at all times. For example I provide reassurance to client and attend to their needs,
communicate with client and constantly watching out for changes in client condition.

E.W. : Sadiq also understands that the nurse does not get the orthopedic patients up for the first time. This is a physiotherapy

Sadiq has met this outcome by giving examples from his clinical practice. He has developed competency over the course of this semester
and can safely provide nursing care for two patients, with support from the staff and clinical teacher when new situations and new learning
arises and he needs guidance.

0800 every shift as required by hospital policy. I also document when I notice any significant changes in cardiovascular,GI, GU or give any

Sadiq has met the Communication outcome and has given examples from his clinical practice. He has become proficient with Meditech,
and giving report to his nurse and clinical teacher at the appropriate times. Sadiq is a quiet student but when asked, he demonstrates much
knowledge and understanding of nursing theory. He communicates well with all his clients and makes every effort to collaborate with staff
regarding the care for his patients.

S.A.B. : I always educate client on deep breathing exercise, coughing, and leg exercise to prevent deep vein thrombosis. I witnessed a
health teaching for client who was placed on heparin subcutaneous injection and was going to be discharged. The health teaching involved
educating the client on how to safely administer the injection on a ball and safe disposal of the injection needle.

procedure and I always seek consent before any procedure is done. For example, some of my client preferred to be bath right after they
have eaten their breakfast and some prefer to have their medications ready just before eating their meal.

example, I gave a report on the operating room experience I attended and provided time to address questions being asked.

IV priming and working with a plum pump, safely administering subcutaneous injections and wound dressing change.

nurse for example I consulted my nursing instructor on how to mix an vial of solute with an Iv bag.

pedal pulse.

that RN.For example I gave the staff nurse the responsibility of doing the heparin monogram of a client as I had no experience with this
process which involved taking the aPTT reading and Inr of client.

S.A.B. : I do prioritize and assess which of my client is need of immediate attention. For example I managed two clients in which one client
required an IV medication and the other client needed a bath. I had to attend client with the medication as bed bath is not of priority for me
at that time.

assessments in the morning and has carefully checked the kardex for the care plan for the day. Sadiq has been careful to collaborate with
his nurse regarding the care planned for his clients so that all care is done well and according to the plan. He completed three satisfactory
concept care maps in this clinical.

S.A.B. : I always work with the physiotherapist and occupational therapist by giving the client the required dose of pain medication in time
before they start ambulating the client as not giving the pain medication will delay the physiotherapist in carrying out their duties. I also
work in close proximity with the staff nurse and my nursing instructor to determine and assess for changes that need to be implemented in
client care.

Sadiq has met this outcome and has provided examples from his clinical practice. He submitted two learning plans, one from last semester
and one for his next semester with goals to improve IV pump management, wound dressing changes, and effective communication with
clients. He also submitted a self-assessment and did and received a peer review. Sadiq was open to receiving feedback from his clinical
teacher and would respond appropriately by improving any concerns the following week.

S.A.B. : I do look up and research on new learning improvement from the CNO such as methods for preventing abuse of client in hospital.
For example one is one too many tool for preventing client abuse.

S.A.B. : I always prioritize nursing intervention and have a daily organisational plan. For example I had to keep an eye on the nasal prongs
of a client because it kept on coming off the client nares each time he changes position. The nursing priority is to make sure that the client
receive the oxygen as ordered because it is one of the basic physiological needs according to Maslow hierarchy of needs.

example, I had to defer the care of a client who required a bed bath and attended to the client who needed an IV medication to start at that

Sadiq has provided clinical examples to support his meeting of this outcome. He gained new experiences every clinical day and with
practice developed satisfactory critical thinking skills, which will continue to develop with more surgical nursing experience. He developed
better prioritization skills throughout the semester caring for two patients. Sadiq always demonstrated professional behavior when in
clinical.

I need to enhance my knowledge,understanding, skills, and judgement by reading my text books and creating a time frame for reading. Also, i have to
make use of the library and open access lab to enhance my nursing skills.

Sadiq has satisfactorily met the nursing outcomes for this clinical placement at Etobicoke General (WOHS) surgical/orthopedic unit. Every day was a
new learning experience for him and Sadiq adjusted well to the weekly changes. With more experience, Sadiq will be able to manage increased
responsibilities within the clinical day and time management and organizational skills will improve. Keep up your keen interest in nursing by asking
questions and reading to gain knowledge. You were always able to answer questions asked in clinical. As you have stated in your learning plan,
practice the skills that you do not feel confident with in the open skills lab at Humber.
I have enjoyed having you in my clinical, Sadiq, and I wish you all the best in your next semester.
