Dear Parents,

I, Hannah James Jupinon of FIN no. G1531061N am currently attending the WSQ
Professional Diploma in Early Childhood Care and Educaton. Under this unit, Unit 9:
Young Children's Language & Literacy Education. I am required to do a short survey
and observation for group of children to meet the requirements of the assessment. I
seek your understanding in allowing me to observe your child and to carry out a
survey with the parents of the child. The questions in this questionnaire is to gather
information on a childs language and literacy background information. The
information will be kept confidential and fictitious name will be used instead.
