1. Specific incident you witnessed or the child has told you: Lyric has recently been withdrawn and uninterested
in class. She says she is contemplating suicide and her mother has been beating her for years. She also says
that her step dad and mother are fighting, her mother is not giving her lunch money, and she is having
nightmares.
2. Location: Private room at school.
3. Severity of injuries: Victim reports bruises on legs and back.
4. Pattern of neglect or abuse: Victim reported that her mother has been beating her for years.
5. Physical evidence: No physical evidence confirmed, but victim states she has bruises on legs and back.

she reported that she feels like the weight of the world is on her shoulders.

suicide is a serious concern and should be addressed immediately.

Observations
1. Specific incident you witnessed or the child has told you: Bouncy came to school and said that his mother
won't allow him to return home. The phone number of his home was no longer in service and his mother is no
longer employed at her listed place of employment.

4. Pattern of neglect or abuse: Last year Dislikes Boy was arrested for allegedly biting Bouncy during an
argument. The victim reports he has been hit a lot since he was 8 or 9 years old.

mother will not let him return home.
