MEDICAL ABSTRACT

This is the case of Reyes, Dionica Flordeliza, 57yo, married, RC , housewife, a

resident of Laoag city, Ilocos Norte, diagnosed as G5P5 (5-0-0-3) Serous
Adenocarcinoma, ovaries, stage III-C.
The history of present illness started 2 years prior to consult as vaginal spotting
for 1 week, using 1 pad/day, minimal-soaked with no other signs and symptoms . No
consult done nor meds taken. 2 months prior to consult, patient had same symptom,
consult done at Bueno clinic. Pap smear was done with a result of atypical glandular
cells of undetermined significance and was advised surgical intervention. Operation
was done and noted massive intestinal adhesions so peritoneal fluid cytology (PFC)
and biopsy were done instead with a result of metastatic serous carcinoma.
Patient was then referred to Dr Cristobal and advised chemotherapy. Exploratory
laparotomy, Radical tumor debulking for ovarian
malignancy ( TAHBSO,
Omentectomy) was also done and tolerated the procedure well. She had 3 cycles of
chemotherapy and tolerated the medication.
Assessment: G5P5 (5-0-0-3) Serous Adenocarcinoma, ovaries, stage III-C; S/P
Exploratory laparotomy, Radical tumor debulking for ovarian malignancy ( TAHBSO,
Omentectomy); S/P chemotherapy 3rd cycle
Plan: For 4th cycle chemotherapy using Paclitaxel-carboplatin
