This is to certify that Milton sangma of 32 years old , male / female residing in
vill/p. o. : khagarpur District. Bongaigaon
Confirmed/ consulted/ treated on 4th April 2106 from 4th April to 19th April
2016 suffering from enteric fever with raised total bilirubin and raised AST i.e
liver ezymes, mild distension of abdomen lassitude & bloating , due to
associated symptoms I considered that a period of absence of duties for 15 days
with effect from 4th April 2016 onward to 19 th April is absolutely necessary for
the restoration of his/ her health Remain/Recommended.
