Referral Resource

Area of need: Unemployment- client states that during his addiction he has had
numerous jobs but would not hold them over a 60 day period, due to attendance or
lack of performance; as a direct result of his alcohol and drug use. Client states he
was unemployed for a year prior to his arrest. Client states he relied on his family
for support until he began stealing from them and breaking into his sister neighbors
homes.
Client will benefit from seeking employment through the community reentry program for veterans, the program will allow the client to learn
skills while maintaining suitable employment. The re-entry program has
direct connections with employers who are contracted to employ
individuals with criminal backgrounds and has successfully completed a
treatment program for alcohol and drug abuse.
Our facility has a contract with the program that allows a Veteran Client to
begin the process of enrollment into the re-entry job placement program
within the last 20 days of the client residential stay. The client will

maintain a negative urinalysis during this period to be accepted in the
second half the re-entry job placement program.
Prior to attending the first session the client will attend all TFAC group
session to assist with cognitive behavioral change, social skills, and
relapse prevention.
The referral is to: The Training Partnership Act United Labor Agency

Area of need: Health Issue Client reports that he was diagnose in 2012 with
cirrhosis as direct result of his AOD use/abuse. Client has no past history of
treatment for cirrhosis due to lack of keeping doctor appointments and following up
with Veterans clinic guidelines that his Medical Case Manager provided for him.
Client will benefit from seeking medical assistance through the local
veterans clinic. Client will receive the necessary medications and
treatment for his Liver condition. Client will be assigned a dietitian to
assist him with maintaining the proper diet.
Our facility will assist the client with making an appointment after his orientation in
the residential treatment program.

Area of need: Housing/Homelessness Client is currently homeless. Client is
currently banned from
Siblings home due to breaking into neighbors home as a direct result of his AOD
use/abuse. Client parents are both deceased.
Client will benefit from a referral to a sober living establishment upon completion of
his treatment program. Client will agree to maintain sobriety and attend 3 to 4
meetings weekly, as one the sober living establishment requirements.
Our facility will assist the client with the necessary transformation from treatment to
establishment process, after successful completion of his treatment program.
