Timesheet Instructions

Timesheets must be on time and accurate

Timesheets must be turned in no later than 5pm on the timesheet due date.
Employee must sign the timesheet.
Supervisor must sign and complete evaluation on the timesheet.
Timesheet must be kept at the worksite in a designated place.

Timesheets must be neat & easy to read

Use dark ink ONLY.


DO NOT USE Liquid Paper or White Out on timesheets.
Timesheets must be neat and readable. If your timesheet, is not readable you will be asked to turn in a new one.
If you make a mistake on your timesheet: Draw a single line through it, make the correction, and initial nearby.

You must work your scheduled shift

Work schedules may not be changed without advance approval from your program case manager
Write your weekly work schedule on your timesheet.
If you will be absent from work: you must inform your program case manager and worksite supervisor as far in advance as
possible.
Provide a short explanation for all missed days in the Reason for Absence column for the date missed.
You must get approval from your program case manager before making up missed hours.

Taking breaks

If you work more than 5 hours in a shift, a 30-minute lunch break must be taken. Lunch breaks are unpaid.

Your lunch break must be shown on your timesheet in the lunch break column by writing, 5
Talk to your worksite supervisor about when to take a short 5-10 minute break during your workday.

Holidays

If your regular work schedule falls on a state holiday, DO NOT WORK. Write the number of hours you are scheduled to work
that day in the holiday column.
If your regular work schedule does not fall on the holiday, you cannot claim the day on as a holiday on your time sheet.
