1 Session = $30.00 and includes a LOMIRA LIONS SHOULDER BAG and CAMP T-SHIRT
2 Sessions = $45.00
3 Sessions = $55.00

I enter my son in the LION HOOPS SUMMER CAMP and accept full responsibility for liability
and cost of treatment for injury to the above registered person. I release the coaches and Lomira
Public Schools from all liability and agree to hold them blameless for any injuries and/or damages
my child may sustain. I also give permission to seek medical treatment for my son if the need
arises.

EQUIPMENT NEEDED:
Basketball shoes, shorts, sweat sock, and T-shirt.
It is suggested that you leave all valuables at home,
Since the camp will not be responsible for any lost
articles.
Basketball will be provided.
TIMES:
2:15pm-4:00pm
GRADES 4, 5, 6
LIABILITY WAIVER
The form must be filled out before players will be
allowed to participate. The school and coaches will
not be responsible for injuries.

FACILITIES
The Lomira High School Gym will be used for camp.
