RECONSIDERATION FOR REFUND OF MEDICAL EXPENSES
This is to ask for reconsideration of approval of the refunds due
me from expenses accrued from the ill health of my daughter when
she had multiple fractures from a fall on 1/10/14 and was admitted
in this hospital for two weeks.
The name in my file is Charity Onyowoicho Odihi while my
children have Oloche as their surname. This has been so right from
the records of my four (4) children in the personal profile of staff that
was filled at the resumption of duty in the hospital more than two (2)
years ago (13/02/12).
Please, note that my husbands name is Oloche Odihi. I use
the same surname with him (Odihi) while our children use his first
name (Oloche). Our registration card at the Staff Clinic bears Odihi
for my husband and I and Oloche for our four children and we were
given registration numbers as 0213A, 0213B, 0213C, 0213D, 0213E
and 0213F respectively in the clinic.
This plea has become necessary because my medical claim in
respect of my daughter Akondu Deborah Oloche(0213F) has been
turned down on the grounds that my children do not bear their
grandfathers name (Odihi) which I use alongside my husband.
The facts stated may be crosschecked so as to clear me to get
these claims Sir.

Attached herewith are my personal profile of staff as submitted at


assumption of duty (13/02/12) and the three forms containing
recommendations from the BSUTH Staff Doctor and my Head of
Department.
I wish to appeal for reconsideration for payment of my
daughter (Akondu Deborah Oloches) medical expenses.
