You stated to Mr. Wright that Youth® kept moving his hands from behind his back. ‘You further Stated, as you were talking to Youth*, he moved ons of his hands fom the Incident Report about Youth® moving his hands. Mc. Wright provided a Memorandum of Concem dated September 21, 2006 to Tom Gilsder, Commissioner's Staff Assistant, stating that you had felsfied the Incident Report on Youth* dated September 6, 2006 by waiting, “! Ar Iindhane approached Youth* and he dropped hls hands and step toward me.” The video showed that Youth® did not step toward you nor did be move his bands, Further, Ms. Prince and Ms. Deiley overheard Youth*teting you tha you “were breaking his @ilNg arm.” Ms, Dalley overheard you make the comment, “Maybe I will” Your comment to Youth® constituted a verbal est to him. Based upon the video reviewed, Youth® did not move either of his hands in a threatening manner and further depicted you telking forcibly to Youth, abrasion to lower lef side with no bleeding noted. Scratches and abrasions to right top shoulder, right arm and noted old sear, sllght bleeding noted, Red half centimeter knot to right side of head. Complaints of head hurting, {wo Ibuprofen given; complaints of te® arm pain. This is a violation ef Department of Juvenile Justice Policles #102, “Employee Code of Ethics"; #104, “Employee Code of Conduct and #208, “Youth Rights”. Furthermore, you recelved a supervisory conference on April 13, 2006 fer failing to docament on the MAR.
