CLINICAL SUMMARY

Date: June 2, 2014

This is a case of Robert Fleming, 64 years old, single, from 18 Juno St.
Bel-Air Village, Makati City. He was diagnosed to have blunt abdominal trauma
from the impact of the punch thrown at him, a direct blow to his abdomen; and
traumatic brain injury from his fall. He is currently in a vegetative state and
retains only 40% of consciousness. He has been given a score of 1 for not being
able to open his eyes at all.

To improve his quality of life, he will be monitored regularly for any

changes.

This certification is given to Robert Fleming, represented by his wife,

Anabelle Fleming, for whatever legal purposes it may serve but not limited to use
for second opinion as such for seeking other medical options.
