BAB IV

DISCUSSION
Congestive Heart Failure is condition in which the heart cant pump
enough oxygenated blood to meet the bodys need. One of CHFs etiologies is
Heart valve disease which is form Rheumatic Heart disease. The os experiences
this condition from 3 years ago but it wasnt diagnosed.
Congestive Heart Disease may have some symptoms such as shortness
of breath during rest, exercise, or while lying flat, fatigue and weakness, nausea,
abdominal pain, loss appetite and persistent cough that can cause blood-tinges
sputum. The symptoms that experienced by os such as shortness of breath, fatigue
and weakness, nausea, abdominal pain, loss appetite and persistent cough.
A confirmed case of CHF requires both clinical manifestation and
echocardiogram imaging. The result of echocardiogram imaging indicates that the
os suffers from congestive heart disease et cause Mitral regurgitation, Mitral
stenosis, Aorta regurgitation, Tricuspid regurgitation and pulmonic regurgitation.
Severity of CHF is divided into 4 according to NYHA, I: No limitation
of physical activity, II: Slight limitation of physical activity, III: Marked limitation
of physical activity and IV: Unable to carry on any physical activity without
discomfort. The os must get inpatient care because of the severity that he suffered.
He was at NYHA IV.
The goal of heart failure treatment is to improve quality of life such
treating underlying condition, controlling risk factor. Prescribe medication such as
Dobutamine as inotropic was used to help the heart to pump enough oxygenated
blood to meet the bodys need. Diuretic such as furosemide and aldactone was
used for reducing congestion as the complication of CHF.
Patient with untreatment CHF can get complication such as Cardiac
Cachexia, Impaired Kidney Function, Congestion (Fluid Buildup), Angina and
Heart Attacks which are can lead to death. The patient was found out has
congestion and was given Diuretic for prevent further complication.

The prognosis of CHF was poor, but the treatments for the damage
valves recommend getting surgical to prevent the further complication.
The patient discharged at (tanggal brp) because the condition was
stabilized and the congestion was fixed. And the patient got advice to repairing the
valves condition with surgery.
