Dear Mr. Cerillo:
This is to request your good office to accommodate the herein listed student(s) of ACLC College
as trainee in your office.
1. Chaddie Estudillo
The trainee(s) is expected to conform the following:
1. Required to report in business attire for the whole duration of the training period of 460
hours.
2. Expected to abide by the rules and regulations of your company for the duration of
training period.
3. The office reserves the right to terminate the training for reason of misconduct or for
reasons that may render the students unfit for training.
4. The trainee shall not be entitled to any benefit whatsoever, unless such benefit shall be
given voluntarily by your company.
5. The trainees performance shall be evaluated by your office after the training period.
We would appreciate it if the trainees shall be given the change to be exposed in various offices
activities or functions of your choice which you deem appropriate for them.
Furthermore, may we also request that they be provided with a Daily Time Record (DTR) card for
monitoring and record purposes.

program for our students.
