Katsucon 17 Commitment Form

I, __________________, having signed this form proclaim myself

committed to the Kutztown University Anime Club’s trip to Katsucon 17 at
the Gaylord National Hotel and Convention Center in National Harbor, MD
during the weekend of February 18th to February 20th of 2011.

By signing this form I agree to acknowledge and abide by the rules and
regulations as set forth by the Kutztown University Anime Club and it’s
officers including but not limited to the following:
1. I agree to pay my share of con fees including my own registration, my
share of the hotel fees for the weekend (divided evenly among all those
attending), gas and parking fees for the driver of my carpool, and all other
personal expenses such as food and/or merchandise that I wish to purchase
during my stay at the convention.
2. I take personal responsibility over my actions during the convention.
3. Should any action I commit result in my removal from the convention or
the hotel then I am responsible for finding my own way home.
4. I will refrain from using any illicit drugs or consuming alcohol (if I am
under 21) during my stay at the convention.
5. I acknowledge that although I am traveling with the Kutztown University
Anime Club I as an individual do not represent the club or the university in
any way during my stay at Katsucon. I am responsible for all of my actions
which reflect only upon myself and will in no way hinder or deface the club
as the whole.
6. I acknowledge that sufficient information has been given regarding
Katsucon including dates, travel information, price and fee information, due
dates for my share of the fees, and any other information regarding the
event.
7. I acknowledge that sufficient information, deadlines, and warnings about
the event have been given numerous times in the form of announcements
and e-mails by the Sergeant-at-Arms and other officers of the Kutztown
University Anime Club.
8. I understand that after the date of January 21st, 2011, if I should
withdrawal my participation in the club trip to Katsucon then any and all
fees I have previously paid up until that point CANNOT AND WILL NOT
BE REFUNDED TO ME in order to prevent the fluctuation of prices and
fees for all others who are attending AND HAVE COMMITTED TO
PAYING THE FEES (DIVIDED EVENLY AMONGST THEM).

I, having signed this form, I am aware of the legitimacy of its content

provided through its notarization which authenticates it and through its
adoption and acceptance by the Kutztown University Anime Club officers as
well as Kutztown University itself.
