1.0 Intern and Campus Liaison/Faculty Advisor

If your campus needs your grades earlier than five weeks after the end of the semester, quarter or term, you must arrange for
your campus liaison to complete this form and return it to The Washington Center. When this form is received, your TWC
program advisor and your course instructor will be notified.

Your program advisor will notify you of the date your grades are due to your school and the date by which you need to submit
your materials to him or her. You must follow-up with your course instructor and make arrangements to complete assignments
early so that your grade can be submitted on time.

It is the INTERN’S RESPONSIBILITY to ensure that:

• He/she completes the assignments necessary in time for his/her grades to be sent to his/her college/university.
• All outstanding balances for which the intern is responsible have been paid.
• This form is sent by the liaison to The Washington Center by the deadline.
