We, the students of BSN 4 RP-I of Brokenshire College conducting a research entitled “Test
Scores, Attitude and Performance of Third Year Student Nurses in Administering Medication”.

In connection to this, we would like to ask for your consent in allowing us to go to the different
hospitals here in Davao City where third year students assign for their clinical exposure for the
purpose of distribution of our research questionnaire to the students who perform administration
of medication via parenteral route with the performance evaluation tool answerable by their
respected clinical instructor.
