PERSONALITEE is a personalized service for t-shirt and hoodie printing. Focused on bringing you convenience at highly subsidized rates, our
service allows you to print group/class t-shirts/hoodies (minimum 25 pieces) in three simple steps. All profits will be donated to
charity!

Step 2: Submit your design

Submit this order form to Wu Wing Yau (09S7B, 97603183) or Nicole Lim (09S76, 91727973) and email your design to:
personalitee.hc@gmail.com by 21st May 2010, Friday. Although the prices quoted are for 2-3 colours only, you can always ask for quotations
for designs with more than 3 colours. Minimum quantity is 25 pieces, but smaller quantities will still be entertained at a different set of
quotations. Do drop us an email if you need more time to submit your design as well.

Upon submission and confirmation of your design, an email containing instructions on when to make payment will be sent.
