The patient reported acute pain from tissue trauma and incisional discomfort after surgery. An initial nursing assessment found the patient to be restless, irritable, pale, and weak with a facial grimace and reported pain level of 7/10. The nursing plan was to change the patient's position, provide comfort measures, assist with breathing techniques, and provide a quiet environment to reduce the patient's pain to a level of 3 after 4 hours. After 4 hours of nursing intervention, the patient's pain had lessened and was reported as a 3 on the pain scale.
