i. To complete Industrial Attachment in ONLY ONE

company.
ii. Benefit / compensation package will follow respective
company HR policy.
iii. Should candidate obtain company permanent
employment offer from other company, candidate
MUST obtain approval from
a. PSDC
b. Industrial Attachment company
before accepting the offer.
 1. Industrial Attachment

iv. Candidates will no longer receive allowance should

candidate obtain permanent employment.
v. During Industrial Attachment, candidates can receive
permanent employment offer from Industrial
Attachment company. Allowance will not be paid to
candidate upon that.
 2. Survey/Kajian

2 months
Kajian Pertama •Fill-in & send survey form to PSDC via softcopy
•To submit 2 months after completed Industrial Attachment

4 months
Kajian Kedua
•Fill-in & send survey form to PSDC via softcopy
•To submit 4 months after Submission of ‘Kajian Pertama’

Allowance RM 100 Allowance to be reimbursed into CIMB

Account Upon Submission of both survey forms

i. Industrial Attachment Evaluation Form

• Page 1 – Industrial Attachment company to
acknowledge & submit a copy to PSDC.
• Page 2 - 4 – Industrial Attachment
company/candidate to submit to PSDC upon
completion of Industrial Attachment
Evaluation feedback to be filled-up by
candidate’s direct supervisor/HOD/HR
personnel.
 3. Forms/Record
ii. Project Status Form
• To be filled-up by candidate’s direct
supervisor & submit to PSDC upon completion
of Industrial Attachment.
• Only applicable for those under Industrial
Attachment.
iii. Offer Letter
• Industrial Attachment company/candidate to
submit a copy to PSDC.
iv. ‘Kajian Pertama & Kedua’ - Refer item 2
