Donation Request Form 2010-2011

This form should be completed in full by all individuals or parties seeking funds from the Student Government
Association. It should be submitted in typed format only. The funds allocated will be for use during the 2010-
2011 academic year.
Please read the directions carefully, so that we can be sure to receive your request and support your

This form in no way constitutes our agreement to participate in any events, this is not a contract
and all donations will be confirmed. You will receive email confirmation within 48 hours of the
General Session, where the request was debated.
