TEACHING PLAN FOR COMMUNITY ACQUIRED PNEUMONIA

DESCRIPTION OF THE LEARNER: The learner is a patient from SLU-HSH, a -year old male, a factory worker who live in Itogon, Benguet. The client has been
diagnosed of Community Acquired Pneumonia and Hypertensive Crisis. The patient stated that 2 days prior to confinement when the patient’s cough started, characterized as non-
productive associated with colds. There were no associated nausea and vomiting. The condition persisted with dizziness prompting the patient to seek medical consultation, hence,
the current admission in the institution. The client claimed that he finished Elementary and that they use and Ilocano at home but he can also understand a little bit
of Tagalog. The client however prefer Ilocano or in conversations. The client is willing to learn on how to manage CAP-MR and his other present condition;
however, he claims that he lacks knowledge and information concerning these diseases.

DIAGNOSIS: Deficient knowledge: proper management of Community Acquired Pneumonia related to lack of information as manifested by the patient’s claim that he lack
knowledge.

PURPOSE: Patient’s mother and/or her significant others will be corrected of their misconceptions and will have an enhanced and strengthened knowledge about Community
Acquired Pneumonia.
GOAL : After the allotted time for discussion and explanation, client will have a broader understanding of pneumonia.

OBJECTIVE: After 10-15 minutes of nurse-client interaction, client and his significant others will be able to have a broader understanding of Community Acquired Pneumonia,
its classifications, Manifestations, risk factors, Medical management and complications.
