    Dear parent/guardian

    I am writing to you regarding your son/daughter featuring in our A


    level media task. With your permission your child will feature in our
    2 minute horror film opening.

    As a parent/guardian you should be aware that we will be following


    strict health and safety rules regarding your child’s part in the
    scene. We ask that you and your child sign the slip below, for proof
    of recognition that you are allowing your child to participate in our
    task.

    This film is specifically for the A level exam board and will be
    uploaded onto YouTube.
