Introduction
วิธีที่จะควบคุมความเจ็บปวดในทางทันตกรรมที่ใชกันกวางขวางวิธีหนึ่งก็คือการทํา nerve block ซึ่งก็
คือการนําเอา chemical agent ที่เหมาะสม (ยาชา) ไป deposit ไวใกล ๆ กับ nerve ที่ตองการ block.
Nerve ที่ถูก block นั้นไมสามารถนํากระแสประสาท (impulse) ใด ๆ เขาสูสวนกลาง ฉะนั้นบริเวณที่ชาจะ
ครอบคลุมเทากับบริเวณที่เสนประสาทไปเลี้ยง. ทําใหทันตแพทยสามารถทํางานในบริเวณที่ตองการทําไดโดย
ผูถูกกระทํา ไมมีความเจ็บปวด. อีกวิธีหนึ่งเรียก infiltration (supraperiosteal injection) เปนการฉีดยาชาไป
ยังบริเวณที่ใกลกับบริเวณที่ตองการระงับความรูสึก โดยฉีดเหนือ periosteum ยาชาจะซึมผานเยื่อหุมกระดูก
เขาสูภายในของกระดูกไปจนถึงเสนประสาทที่มาเลี้ยงฟน ซึ่งจะเขาสูฟนบริเวณรูเปดที่ปลายรากฟนจึงสามารถ
ระงับความเจ็บปวดจากประสาทฟนได.
ดังนั้นการมีความรูความเขาใจใน anatomical structures ที่เกี่ยวของกับการทํา nerve block injection
จึงเปนเรื่องที่เห็นไดชัดวาเปนสิ่งสําคัญมากสําหรับทันตแพทย. และในการฉีดยาชา, การทราบแตเพียง
ตําแหนงที่จะแทงเข็มเทานั้นไมเปนการเพียงพอ. เพราะเหตุวาดวยขอมูลเพียงเทานี้การทํางานจะไมประสบ
ความสําเร็จเพราะอาจเกิดอาการแทรกซอนไมเปนผลดีตามมาได ผูที่ไดรับการฝกฝนอยางถูกตอง และมี

ไดการทํา nerve block ที่ดี.
นอกเหนือจากความรูเกี่ยวกับ anatomical structures และ relationship ของ structures แลว ตอง
ทราบหนทางที่สั้นที่สุดที่จะไปสู nerve นั้น ๆ และหนทางที่ปลอดภัยที่สุด เปนการหลีกเลี่ยงที่จะไมทําอันตราย
ตอ artery, vein, muscle, gland และ structures อื่น ๆ หรือแมแต nerve เอง.
ดังนั้นในการที่จะฉีดยาชาตองมีความรูหลายอยางประกอบกัน กับตองฝกหัดซ้ําแลวซ้ําอีกเปนจํานวน
หลาย ๆ ครั้ง จนมีความชํานาญเพียงพอจึงจะทําใหการทํางานไดผลดีและไมมีอันตรายเกิดขึ้น.

ความรูที่ไดจากเรื่องตาง ๆ ที่ศึกษาผานมาแลวพอจะรวบรวมเปนหัวขอที่จะตองทบทวนเพื่อให
สัมพันธกับบทนี้ได ดังนี้
1. Osteology of the skull
บริเวณของ bone ของ skull ที่เกี่ยวของกับการฉีดยาชาเฉพาะที่ (local anesthesia) สําหรับฟนและ
supporting tissue ของฟนคือ maxilla และ mandible.
1.1 Maxilla
1.1.1 infratemporal (posterior) surface
Posterior หรือ infratemporal surface ของ body ของ maxilla มีลักษณะเรียบ
และเปน convex surface ยื่นไปทางหลังและทางดานขาง. ที่บริเวณใกลตอนกลางของ surface นี้ปรากฏมี
alveolar foramen หนึ่ง foramen หรือมากกวาหนึ่ง สําหรับเปนทางผานเขาของ posterior superior alveolar
nerve ซึ่งเปนแขนงของ maxillary nerve.
ภาวะแทรกซอนสําคัญที่เกิดจากการแทงเข็มไมถูกตําแหนงของ
1. Posterior superior alveolar nerve block
Haematoma เปนภาวะแทรกซอนที่พบไดบอยในกรณีที่แทงเข็มลึกเกินไปจนเขาไปเขาไปสู pterygoid
venous plexus ซึ่งถาเข็มไมสะอาด หรือแทงผาน structure ที่มีการติดเชื้อมากอน เชื้อโรคก็สามารถผานเขาไป
ยัง cavernous sinus ทําใหติดเชื้อใน cranial cavity ได
ในรายที่มีการแทงเข็มเขาใน maxillary artery และมีเลือดไหลออกจาก artery ดวย จะสังเกตเห็นอาการ
บวมบริเวณแกม และมีกอนเลือดเกิดขึ้นบริเวณแกมใกลกับขากรรรไกรลาง

get a good result.
B. Anatomical information to be reviewed.
1. Skull - maxilla : posterior and anterior surfaces, palate.
- mandible : mandibular and mental foramina, lines and ridges.
2. Location of arteries, veins and others : infraorbital region (facial vein); infratemporal
fossa (maxillary artery); mandibular foramen region (ligament, fascia, raphe). Mental foramen area
(mental foramen); posterior border of ramus area (parotid gland).
3. Location and attachment of muscles (muscles of mastication, muscles of facial
expression).
C. Summary of important nerve supply to the teeth and jaws.
1. Pulps of maxillary teeth; molar teeth, premolar teeth, canine and incisor teeth.
2. Buccal supporting tissue (maxilla); molar and premolar teeth area, canine and incisor
teeth area.
3. Palatal supporting tissue (maxilla); soft palate, hard and soft palate, hard palate
(anterior part).
4. Mandibular teeth : pulps of all mandibular teeth; buccal supporting tissues (molar
and premolar teeth area, premolar and incisor teeth area); lingual supporting tissues.
D. Summary of informations to be reviewed : course, distribution, relationship of maxillary and
mandibular divisions of Trigeminal nerve; function of each branch of these nerves; location of related
muscles; structure of the skull in detail, especially in the areas related to the local anesthetic injection
in dental practice.
