By submitting this application, I am declaring my commitment to join Beta Alpha Psi as a part of the new
member class of Fall 2008. I understand that I will be expected to fulfill all the candidate requirements
and thereafter continue to meet the chapter requirements for continuing members. The requirements as
currently set forth by the Executive Board of the Lambda Beta chapter of Beta Alpha Psi are as follows:

12 Service Hours
12 Professional Hours
32 Total Hours
Regular Attendance of Beta Alpha Psi Chapter Meetings

I understand that should I fail to meet these standards during my candidacy, I will not be initiated into Beta
Alpha Psi. Furthermore should I fail to meet these standards during an academic year while I am an
active member of Beta Alpha Psi, I agree to resign from the organization, return my certificate of initiation,
and forfeit my Beta Alpha Psi pin.
