ACCESS 2011 DONATION FORM

How Can I Make a Donation?

There are several options, just choose the means of


payment that suits you best!

Step One – Please check one of the options below.

1] General donation, the money may be used for all ACCESS 2011
goals_____

2] Purchase of a theatre seat for $250 _______

3] Donation for Snoezelen Room equipment _______

Please Note: When you buy a theatre seat your name or a name of
your choice will be placed on a plaque, which will be attached to a
seat. Please indicate below the name that you wish to appear on the
seat.

A plaque will recognize donations for the Snoezelen Room with donor’s
names on the wall of the room.

General donors’ names will appear on the “Contributor’s Wall” which


will be placed by the entrance of the John A. McDermott Community
Room.
Step Two – Complete the following and mail or drop off to
AHSS with your cheque attached.
