Diploma Information

Please fill in your child’s name exactly the way you want it to
appear on their diploma. This needs to be returned to Mrs.
Tallman in the Matthews Middle School office no later than
March 1, 2011. If this form is not returned by that date, your
child’s name will be printed as it appears on their school
records.
