provided is correct. I/We am/are releasing all right, title and interest in the license(s) I/we possess with respect to the
product(s) (including the right to use any prior versions or upgrades to) in exchange for the right, title and interest in
the equivalent license(s) of Mentum Planet . I/We agree to be bound by the terms of the Mentum Planet End User
License Agreement. I/We understand that by signing and submitting this Software Upgrade Form, I/we am/are
relinquishing all rights to the product(s) and my/our name will be eliminated from Mentum S.A.’s customer records in
connection with the product license(s) identified below and replaced with the equivalent license(s) of Mentum Planet .
If this upgrade is being completed on behalf of a company or other organization, I/we represent and warrant that I/we
have the authority to sign this upgrade on behalf of the current registered license owner.
