Application Guidelines

IMPORTANT: Please read the following instructions carefully before you submit the application. Please
note that Incomplete Applications and late submission will not be processed.

INSTRUCTIONS

1. ASSIGNMENTS:
Attach separate paper(s), do not exceed two pages of paper A4, font TNR, size 12, space 1.5
a. What is your biggest achievement? And what is your biggest failure, and how do you
overcome them? Please elaborate your answer.
b. What is your plan for the next five years?
c. What is your motivation in joining this program?
d. How do you interpret the platform “GET INVOLVED! Indonesian youth for global issues”?
e. Which current issue in Indonesia that concerns you most? Please give your comments and thoughts.
f. How would you like to make contribution towards the society upon the completion of the exchange
program?

2. SUBMISSION:

Hard Copy:

Before submitting the applications, please write “Seleksi PPAN Jawa Timur Tahun 2011” in front
upper-left side of a sealed brown envelope.
Use the application checklist for the cover sheet.

Please mail your application and all supporting documents (through the Postal Service) to:

Soft Copy:

Once the hardcopy of the application have been submitted to the aforementioned address, please
send an e-mail attaching all the application documents (excluding Photo ID Card / KTP) to
pendaftaran@pcmijatim.org.

IMPORTANT: In the email subject line, please put:

“Seleksi PPAN Jawa Timur Tahun 2011 – [your name]”
Signature in the application form is not required for the softcopy submission.
Official announcements and other relevant information regarding the result of this selection process
shall be published in PCMI Jawa Timur website at this address http://www.pcmijatim.org.

DEADLINES: All applications and supporting documents both hard and soft copy must be received by:
March 22nd, 2011 at 10 AM.
