    Exact Location : On road- in between HLSB & FO Tank

    Date & Time : 24/1/11 & 02.45 PM

    Nature of Injury : Injury on his waist (backside)--minor

    Brief Description of the accident:

    On 24/1/11 at around 2.45PM, Mr. Kesab Barik (Victim) who was working as a Rigger in M/s Effort
    Erectors met with an accident when he was tying a sling to a piece of pipe(FO pipeline/3 inch dia & 12ft
    long) lying on ground for shifting, after fixing the sling he signaled the hydra operator Mr. Jagannath
    Rout to lower the front hook sitting right below the hook, suddenly the hook slipped from the rope & fell
    on his back .Immediately he was taken to Danagadi hospital for treatment, doctor confirmed that there
    was no injury but just a pain due to the impact, then he was given medicines & sent back for rest.

    Root cause of the accident: 1. hydra hook was fixed with only one U-clamp.
    2. Sitting right below the hook before taking the hook
    into position.

    Preventive measures to be taken to avoid the re-occurrence :

    1. The said hydra (AP31AT-5868) should be tested by competent person immediately.


    2. Experienced operators & riggers must be engaged with strict supervision.
    3. Carelessness must be avoided during working.
