DRUG STUDY

METOCLOPRAMIDE (reclomide)

Classification: gastrointestinal stimulant

Pregnancy category: B

Action: dopamine antagonist that acts by increasing sensitivity to acetylcholine; results


increase motility of the upper GI tract and relaxation of pyloric sphincter and duodenal bulb.

Indication:

For the treatment of gastroesophageal reflux disease (GERD)

Contraindications:

-Metoclopramide should not be used whenever stimulation of gastrointestinal motility might


be dangerous, e.g., in the presence of gastrointestinal hemorrhage, mechanical obstruction,
or perforation. 

-Metoclopramide is contraindicated in patients with pheochro-mocytoma because the drug


may cause a hypertensive crisis, probably due to release of catecholamines from the tumor.
Such hypertensive crises may be controlled by phen-tolamine. 

-Metoclopramide is contraindicated in patients with known sensitivity or intolerance to the


drug.

-Metoclopramide should not be used in epileptics or patients receiving other drugs which are
likely to cause extrapyramidal reactions, since the frequency and severity of seizures or
extrapyramidal reactions may be increased.

side effects:

change in mood (depression), constipation, diarrhea, drowsiness, fatigue, involuntary


movements of the limbs or eyes, restlessness

Adverse Effects:

Depression( with suicidal ideation), seizures, agranulocytosis, hypotension,

Nursing responsibilities:

 Inject IV slowly over 1-2 mins to prevent transient feelings of anxiety


 Note reasons for therapy
 Assess abdomen bowel sounds, distention, N&V

Client/ Family teaching:

 Instruct to take as directed may dilute syrup in water, juice or carbonated drink
 Advise that drug increases movements or contractions
 Instruct to avoid alcohol and CNS depressants
