TIME: Registration at 7:30 a.m. First Tee-off at 8:00 a.m.
PLACE: Belmont Hills Golf Club, 25 Belmont Hills Drive, Warwick, WK 06

ENTRY FEE: $155.00 per golfer or $620 for a team of four (4) (includes: green fees, cart and catered lunch) Payment Due: June 3, 2011

Cheques should be made payable to New Creation Worship Center. Submit completed forms via email to women2011info@gmail.com.
A Committee member will contact you to confirm your registration and fees.

Note: Early-bird registration of $145.00 per golfer or $580.00 for a team of four (4). Deadline: May 16, 2011
Mulligans will be on sale for $10 each. (Maximum of 3 mulligans per person.)

Online registration available at www.interwomenconference.org or submit completed forms
Cancelation Policy: If golf tournament is canceled due to weather or other events, player will receive a full refund
If player cancels, 48-hour notice is required to receive 50% of entry fee
If player is a “no-show”, entry fee is non-refundable.
