We refer to the above matter. We act for Pacific Engineering Sdn. Bhd., former employer of Cheang
Sin Fook.

Sometime in Jun 2007 you issued 2 sick certificate to Cheang Sin Fook, both bearing certificate No.
Attn: DR Ng Wai Keong
154798, a copy each of which is enclosed herewith for your kind attention.

Please confirm with us the following:-

a) that the 2 said certificates were issued by you to Cheang Sin Fook;
b) the 1st certificate did not contain the word “as hospitalization”, and you did add the word, “as
hospitalization” in the 2nd copy of the certificate, some 3 weeks later (as received by our
client;
c) the reason as to why “ as hospitalization” was added in subsequently by you.
d) You issued the certificate on 18/06/2007, but you did certify Cheang Sin Fook was unfit for
duty on 15/06/2007 onwards…. Could you provide reason for granting leave?

As the above matter is now subject to a labour case claim before the Industrial Court, please let us
have
your early reply. Your urgent reply would assist us in assessing whether to call you to give evidence
in
Court or otherwise.
