    DOCUMENT CHECKLIST FOR THE LEGALIZATION PROGRAMME 2011

    1. Photocopy of COMPANY REGISTRATION or BUSINESS REGISTRATION


    - Form 9, 24 & 49 and Memorandum & Articles (M&A) or Form B & Form D

    2. Photocopy of Identity Card (NRIC) ALL DIRECTORS OR PROPRIETOR

    3. LICENSE issued by LOCAL AUTHORITY / MITI / CIDB / POLIS etc. (whichever relevant)
    e.g. Business License/Lesen Perniagaan (MPS, MPSJ, DBKL etc.)

    4. WORKERS PASSPORT SIZE PHOTO with BLUE BACKGROUND (4pcs.)

    5. COMPANY LETTERHEAD (8 COPIES)


    Procedure Guideline for Legalization Programme 2011

    1.) Collecting all documents, see Checklist, and Application and BioData
    Forms.

    2.) Collected documents forwarded to KDN for Registration, Chopped


    Application Form received by Immigration, due payment RM 500
    deposit.

    3.) After receiving the Yellow IC, the worker has to come for Finger
    Printing, then Passport will be applied with the relevant Embassy and
    2nd payment is due as following by sectors:
    Services: RM 2,500
    Construction and Manufacturing: RM 2,000
    Plantation and Agriculture: RM 1,000

    4.) After received Passport, doctor visit for Fomema (if unfit, all fees are
    non-refundable) and Insurance will be applied and the balance
    payment is due.

    5.) Done, worker is now issued with a valid working permit.
