Gadia at regal age, Filipino and resident at Diam, Valenzuela after having been duty sworn to in accordance with the lat o do hereby depose and state; 1. That I am the holder of a LIBRARY CARD issued by the Manila Central University- Caloocan; 2. That sometimes lately the said LIBRARY CARD was lost/misplace by the under circumstances not yet known to me; 3. That I executed diligent effort to locate the said lost above the mentioned lost not to avail; 4. That I executed this affidavit to attest the truth of the above for going facts for all legal intents and for what so ever legal purpose this document may serve.
