PERMIT TO WORK PROCEDURE Do I need a Master Permit to Work (MPTW) ?

Yes, all work requires a MPTW Does the activity involve an isolation or hazardous activity? Yes

You will also need a separate Permit for the following hazardous activities Confined Space Work at Height Critical Lift Excavations Lock Out Tag Out (Isolation)

NUKEM Engineer to use and fill out relevant Permit Attach Technological Card/Method Statement/Risk Assessment Send to HSE Department for Review, Inspection of area and Sign Off If documentation is not complete return to NUKEM Engineer for corrections If documentation complete then return to NUKEM Engineer with signed off Permit NUKEM Engineer will then provide paperwork to Contractors Supervisor in charge of the work and obtain his signature on the permit Contractors Supervisor is then responsible for briefing his operatives on the content of the Permit and any hazards involved in the work

Contractor completes the work in the agreed timescale or requests a Permit Extension

Extension Required

Contractor Supervisor & NUKEM Engineer to sign off work as complete and satisfactory

HSE Department to inspect area and conditions before signing extension

Engineer to give completed signed off Permit to HSE Department

HSE Department gives Permit back to Contractors Supervisor to carry on works

HSE Department to sign as complete and file away for record purposes
