MUST Report

In a meeting this morning with the Civil Aviation Authority (CAA),you were informed by them that the company as an operator of 9V-PLM has contravened Air Navigation Order Paragraph 88 with regards to Mandatory Reporting. The company has failed to inform the Authority within 72 hours of a reportable incident. In this case, the incident highlighted by CAA occur one(1) week ago when the No.1 engine of 9V-PLM thrust suddenly cannot be pushed beyond flight idle on landing approach to Bangkok Airport. The problem was solved by the replacement of No.1 Engine (RR AE3007C1)fuel pump. The CAA now requires the company to provide 1. Immediately a written report of the incident 2. Corrective Action Plan (CAP) to ensure the company comply with the mandatory reporting requirements. As VP Operation you will need to response to CAA request. Before you are able to do that, you will need to 1. Find out the structure of the required report 2. Understand the requirements of mandatory reporting
